Cancellation Policy
We understand that unanticipated events happen occasionally in everyone’s life. In our desire to be effective and fair to all clients, the following policies are honored:
24 hour advance notice is required when canceling an appointment. This allows the opportunity for someone else to schedule an appointment. If you are unable to give us 24 hours advance notice please call me to discuss. But, please be prepared to be charged the full amount of your appointment. This amount must be paid prior to your next scheduled appointment.
No-shows
Anyone who either forgets or consciously chooses to forgo their appointment for whatever reason without contacting me will be considered a “no-show.” They will be charged for their “missed” appointment.
Late Arrivals
If you arrive late, your session may be shortened in order to accommodate others whose appointments follow yours. Depending upon how late you arrive, your therapist will then determine if there is enough time remaining to start a treatment. Regardless of the length of the treatment actually given, you will be responsible for the “full” session. Out of respect and consideration to your therapist and other customers, please plan accordingly and be on time.
HIPPA
We honor HIPPA regulations as far as client confidentiality. I will be maintaining full records for seven years after the last date of service or for three years after a minor patient reaches majority, whichever comes later.
Confidentiality
I may discuss examples for a specific treatment with another client but will never mention a full name unless client has given permission to do so.
Photos
I may take pictures of sessions at the clients request but only to keep in the clients records or share that information with the client unless a waiver has been signed. If you are needing a photo waiver, one will be available at time of service.Payment & Fees
Payment
Payment for services and products is due at time of service. Gratuity is not included in the massage price. Gratuities are appreciated in cash upon completion of the massage. I accept Cash, Check, Cash App, Venmo, Zeele, PayPal & Square for services & in person sales. Online sales is PayPal.
Terms
Once you make an appointment every effort will be made to ensure your safety, security & comfort. If at any point before, during or after your session you are uncomfortable please speak up and make me aware of the issue.
I am not a medical Doctor and am unable to diagnose issues. I will follow Doctors and Physical Therapists orders for synergistic therapy.
It is your responsibility to inform me of all pertinent medical conditions that may affect your session. Failure to do so may result in an undesired session or complications. With this knowledge Elemental Holistic Massage is not liable for unknown conditions. If you are Pregnant you MUST make me aware prior to scheduling a session. If it is not disclosed prior to session and it is discovered during session- Treatment will stop immediately and you will be required to sign a waiver absolving Elemental Holistic Massage from any damages that may occur post session. I do my absolute best to keep my clients safe.
